Drs. Chin, Liu and Patel have established practices and can only take a limited number of new patients at a time. New patients require more time than established patients because the doctors need to learn their histories and document everything. We have a wait list that is in the 400s. We have decided to stop taking further patients for the wait list. We encourage you to keep looking and kindly let us know if you no longer need to be on our wait list. We understand the difficulty in looking for a family doctor during a primary care shortage. Please have patience as we do our best to expand our capacity to care for patients in a safe manner.

Medley is the patient portal with links to appointment times and messages from our office. This is different from our old messaging system.
If you have never signed in to Medley before, please visit the Avaros website link below for instructions on how. Do not use your pin - it does not work.

Clearly state your MOST URGENT issue to address.
Children with illnesses are targeted to be seen within 1-2 days
Time sensitive issues like pain, worsening mental health, and infections are targeted to be seen, within 2 weeks depending on the urgency determined by our triage process.
Preventive health checks, “need a referral”, forms and “need an MRI” will be booked at intervals
4 weeks or longer.
No Shows and Last minute cancellations contribute to longer wait times. Give us 24 hours notice if possible so we can free space for those who need appointments.
No news is NOT good news. Always make sure you get your test results.
You will be called for an appointment to review test results or you will receive a message via secure portal
Call the office to see if we have received your specific test results, if you have not heard from us within 2 weeks of test completion. Only a clinician can give results to the patient directly or via the administrative assistant with instructions.
Prescription renewals will not be booked urgently. You have 4 options to renew your medications:
Follow up with your clinician when they asked you to (e.g. every 3-4 months for diabetes, every 6 months for hypertension)
Book an appointment at least 4 weeks before your medications run out.
Renew a prescription without appointment, at the clinician's discretion, for a fee of $25
Contact your pharmacist to refill a short course of your prescriptions for a fee to the pharmacist
For urgent issues, you should hear within 2 weeks
For non urgent issues, you should hear within 6 weeks.
Contact the provider of the service (specialist or special testing centre) if you do not hear within the specified time period.
If there is a problem after that, contact us.
Some issues are best served going to the ER for time sensitive testing AND observation that is impossible to deliver through our office. Administrative assistants advise based on their triaging rules or clinician advice. If the administrative assistant tells you it is the clinicians advice and your refuse to go, it is going against medical advice.
Please arrive at least 10 minutes prior to your appointment, so that if we need to check your blood pressure, weight, etc, this will not take up your appointment time with the Doctor.
If there is space in the schedule, we can accommodate patients being late up to 5 minutes. But if we are fully booked, we cannot. Re-booking is not a punishment, it is what is best for all involved.
To respect everyone's time, please show up early! :)
During certain seasons (like cold and flu season), we will implement mandatory masking to protect our staff and patients. Sick people come to our office - with coughs, fevers, etc. We want to protect you.
Sick clinicians and administrative assistants lead to clinic shut downs and patient inconvenience. We want to protect our vulnerable patients (cancer patients, newborns) who have no option but to come to us for healthcare.
In our office, the rights of the individual do not trump the rights of our clinician, administrative assistants, and other patients.
Our Clinicians are not licensed to practice outside of Ontario.
Not all services are paid for by OHIP. Just as we pay lawyers, hair dressers and plumbers for the time AND expertise, we ask to be compensated for our time and expertise. We always make exceptions for people who are in need.
Have an honest conversation with your clinician. We want to do what is best for everyone involved.
